LORT and Silmarillion question

I am not finished with the Silmarillion, but I am trying to figureout where things are taking place relative to the maps of Middle earth from LOTR.

There are two continents in the LOTR world. (what is LORT?) One is Middle Earth and one is the Undying Lands. The Undying Lands is where all of the elves are headed to and where the Valar live. It is also where the two trees are/were. There is also a place called Belariand(?sp) that was part of Middle Earth but got destroyed. It was west of the Shire and the Havens but sank in to the sea.
