Looking for new monitor

The 21.5 inch samsung TV I have been using ever since I got my rig is atrocious. It must have a slow response time or is simply defective, as it has severe ghosting.
I want a recommendation for a monitor with the following:
1920x1080 resolution
DVI or DisplayPort input
5 ms or lower response time (if lower than that actually makes a difference in ghosting, then I want 1 or 2 ms.)
Less than $200

Optional, but very nice if present:
Less than $150
Built in speakers
HDMI Input
