Looking for a PCI USB2.0 add-on adapter that has header pins

I have an internal 10-1 reader that I would like to install, see the link to it's product page below. The USB and 1394 cables coming from the reader breakout to individual pins, so that it will adapt to a variety of motherboard USB headers.

Does anyone know of a USB2.0 PCI adapter that has internal header pins?
