Our lab needs new mice, because all the current ones are bare minimum cheapy POS that cramp our hands by the end of the day.

We spend all day tracing tissues on medical images (MRIs and CT scans)...so accuracy would certainly be beneficial. However, I suspect either mouse would be a step up from our current hand-killers. So I'm really more concerned with comfort.

The mx518 kinda looks like my old Cordless Mx Duo, which was one of the best mice I ever used in terms of comfort and functionality...before it died. Now at home I use a MS cordless laser 5000, which is pretty nice too. In our lab's case though, I'd prefer to stick to corded just to avoid any interference issues. Unfortunately most of MS's good mice seem to be cordless.

I assume both will work fine in Linux (opensuse 10.3)

Any opinions/experiences?

I've used the mx518 for a long long time, and it has been great. It's a mouse that you can use with just your fingertips. I've never used the mx400, but it looks like it would sit more in the palm of your hand. I suppose it's up to your preference.

A nice feature about the 518 is that you can adjust resolution on the fly. Might come in handy if you need to slow down for those fine lines.

I like the shape of the Mx518, but of course everyone's different. Try the feel at a B&M store. You could also order a couple of different varieties and see which people prefer. If different people are using the same computer you could even have two different mice for it so they had a choice.

Not sure if this is produced anymore... but as far as comfort goes, I really did like the Microsoft Intellimouse 4.0, it was very ergonomic, and extremely light, although a bit bulky since it's so glove-like for right hands. I really liked this mouse for low sensitivity use, but gave it away to a friend that was in need of a mouse.

I love my mx518. Had it for 2 or 3 years now. Comfortable and accurate. Wouldn't give it up.

I've never used a mx400, but I've had 2 mx700s(a wireless mx518 for all intents and purposes), and I consider them to be the epitome of comfort, especially in that price range. I also like the MX Revolution, but that's considerably higher in price.
