That is a CD image. CD burners can use those 'images' to burn data onto a cd. If you don't have a cd burner, don't worry, you can also use a program called WinImage to extract the file from the .iso image file. Do a search for winimage on the web.

Edit: ok, 99% of the people asking this have a warez iso, but this could be a linux dist.

No, not all iso files are illegal. As somebody else mentioned, linux distributions are generally downloaded as iso files.
