Lets play: What is wrong with this system??

To begin with here are the system specs:
Abit KT7 mobo
Duron 700 oc 850 runs in the low 40's
V3 2000 pci video card
Soundblaster live gamer
Netgear FA310TX ethernet card
2 X 128MB memory pc133
IBM 18GXP HDD, 7200 rpms ATA66
CL 48X CD-rom
Yamaha 8X4X24 CD-R
Windows 98 all updates except directX 8, still using 7a
Generic 17 inch monitor

Next the problem. Was playing the Alice demo and when finished and quit out of the program I heard what sounded like a pop and the desktop looked like it was in safemode, but it was not. It turns out the 3dfx tools disappeared, but only realized this later after I had already done a clean install of windows 98 and all drivers. Than last night after playing Cataclysm for several hours (great game) I quit out of that. This time no popping sound but the icons in the quick launch toolbar and all open windows are the wrong colors. I tried to re-install the video card drivers. I even removed the 3dfx adaptor from the device manager and replaced it with a standard svga adaptor. Then tried re-installing the 3dfx drivers. The funny thing is the icons look right when the standard svga adaptor is used and once the 3dfx drivers are re-installed the icons go all to hell. I was too tired last night to take out the video card and reseat it and don't really want to do another clean install of windows but most likely will. The question is what is causing this problem.

Here are a list of things I think could be wrong:
1) the first pci slot has a problem??
2) the drivers I am using 1.05 are not stable, well I am going to install the latest drivers tonight anyway so this is really not a problem.
3) the I/O voltage is too low, read that cards are more stable at 3.4-3.5 rather than 3.2 or 3.3.
4) Stop playing video games, not at all likely to happen.
