Apparently there is a sound bit from an old western that has been recycled in tons of movies, tv shows and video games over the years. including:

The scream appears in several Peter Jackson films:
In King Kong (2005), during the brontosaur stampede, as a sailor is knocked off a cliff.
In The Return of the King (2003), a Gondorian emits the scream after he is dropped out of the sky by a Nazgul, and a Haradrim emits it after he is thrown off a Mumak by Legolas.
In The Two Towers (2002), an elf soldier gets thrown off the wall by an Uruk-Hai Warrior on wall of Helm's Deep.
The scream can be heard in every Star Wars movie, including the Holiday Special and the animated series Clone Wars.
The scream can be heard in all three Indiana Jones movies.
Cars (2006), during a wreck in the opening race.
16 Blocks (2006), during the bus hijacking scene.
Æon Flux (2005), in a firefight as a Breen soldier falls off a roof.
Fantastic Four (2005), as Doctor Doom battles the Fantastic Four on the streets of New York causing chaos. (time: 1:31:35)
Kingdom of Heaven (2005), a Muslim soldier emits the scream after a trebuchet hits a Muslim siege tower.
Madagascar (2005), when the people are fleeing from Alex the lion in Grand Central Station.
Sin City (2005), when Marv throws a police officer out of a stolen cop car.
Storm (2005), the first Swedish feature to use the scream.
Wallace & Gromit: The Curse of the Were-Rabbit (2005), as the Were-Rabbit snatches Lady Tottington and begins its Kong-like rampage. (time: 1:06:33)
Sky Captain and the World of Tomorrow (2004), construction worker on scaffolding destroyed by bat-like flying robots pursuing Joe and Polly through New York City.
Anchorman: The Legend of Ron Burgundy (2004), in the fight sequence between the news teams, when Champ throws another man into a car windshield. (time: 1:04:34)
Hellboy (2004), when a grenade hurls a Nazi engineer against the portal, just as he is disintegrating.
Team America: World Police (2004), when Kim Jong Il is knocked off the balcony by Gary Johnston.
Harold & Kumar Go to White Castle (2004), when Harold and Kumar are riding the cheetah and Harold is hit by a tree branch.
Agent Cody Banks (2003), when a guard falls off a balcony and through a fiberglass ceiling. (time: 1:25:41)
Kill Bill: Vol. 1 (2003), in the battle at the House of Blue Leaves, twice.
Looney Tunes: Back in Action (2003), when Damian Drake tosses a grenade behind him; the scream is heard when it explodes.
The Animatrix (2003), in the episode Matriculated. Occurs when the Sentinels attack the humans in the lab near the end.
The Majestic (2001), when they first test the Majestic's lit sign and it shorts out.
The Little Mermaid II: Return to the Sea (2000--Direct to video), A crew member jumps from his ship and screams just before part of an iceberg falls on the ship. (time: 57:48)
Small Soldiers (1998), when Brick Bazooka loses his legs to Alan's bike gear.
Dante's Peak (1997), as Dr. Paul Dreyfus dies.
Hercules (1997), when the Cyclops is smashing houses looking for Hercules.
Face/Off (1997), rapelling SWAT team member shot, again when a man is shot as his boat is stolen by Castor Troy.
Die Hard: With a Vengeance (1995), as Zeus and John are driving through the park Zeus asks McClane if he is aiming for the people, to which McClane replies, "No... [an abbreviated scream] maybe that mime."
A Goofy Movie (1995) During the song "On the Open Road," Goofy's car strikes a construction scaffold. (time: 28:11)
Toy Story (1995), when Buzz is knocked out of the window by the table lamp. (time: 27:14)
Aladdin (1992), the villagers let out a Wilhelm scream when Genie lifts up the palace. (time 1:11:23)
Batman Returns (1992), when Batman punches and grabs an enemy and hurls him towards a railing.
Reservoir Dogs (1992), when Mr. Pink pushes past pedestrians on the street after the botched robbery.
Raiders of the Lost Ark (1981), twice; once when a German soldier riding in the back of the truck carrying the Ark of the Covenant falls out of the back and onto Gobler's car, and again when two soldiers are knocked off the side of the truck.
Beauty and the Beast (1991), in the raid between the house supplies against the villains and Gaston. (time: 1:15:13)
The Doors (1991), during the toppling of speakers while the crowd charges the stage in the chaos after Morrison "reveals" himself.
Willow (1988) features two instances. The first occurs as the soldiers' chariot crashes during the chase scene and he is sent flying. The second can be heard when the Brownies trigger the large spear shooter that hits several soldiers.
Spaceballs (1987), when John Candy uses a section of tubes to bounce several laser bolts back at guards, and each of them screams when they're hit - the last one lets out a Wilhelm. (time: 1:01:41)
Robocop (1987), when Ronny Cox's character falls out of a building.
The Wild Bunch (1969), during the last stages of the opening fight sequence.
Them! (1954), at least one sailor uses the scream when attacked on his ship. There are three screams in the scene, only one is definitely the Wilhelm, but the other two sound remarkably similar and were most likely created by the same voice.

Television shows
American Dad, where an agent at the CIA pulls the pin on a grenade and casually tosses it over his shoulder while in the training room. It was also heard in a later episode as two Handicapped Mafia hitmen beat up a rival.
Doctor Who, during the "TARDISODE" (made-for-Internet prequel trailer) for the 2006 episode "Tooth and Claw", as a man is attacked by a werewolf.
Drawn Together, in the episode "Dirty Pranking No. 2" when someone gets blasted with a robo-fly's raygun, in "The Lemon-AIDS Walk" when Captain Hero takes steroids, in "A Very Special Drawn Together Afterschool Special" during a barroom brawl, and in "The Drawn Together Clip Show" when the the Producer opens fire on the audience.
Invasion , when a man is thrown over the edge of a pier during the series finale (May 2006).
The League of Gentlemen, on the main menu of the second series DVD, accompanied by the show's theme tune.
The Life and Times of Juniper Lee, in almost any, if not every, episode.
Robot Chicken, in the episode "Nutcracker Sweet" when Walt Disney's severed head with it's giant robotic-spider body invades Cuba and shoots down a Cuban fighterjet.
Family Guy, in the episode "North by North Quahog" when Lois and Peter try to get away with Mel Gibsons new movie from two catholic priests they drive through a mall and they manage to drive past a piano but the priests drive right at it and the man at the piano jumps out of the way.

God of War, when you kill the guy holding the bridge on the Rooftops of Athens.
Mortal Kombat, if a player uses Johnny Cage to defeat Goro, the villain emits a Wilhelm scream.
Spider-Man 2 (2004) for the Nintendo DS, when the player throws an enemy off a building.
Lego Star Wars: The Video Game (2005), either Obi-Wan Kenobi or Anakin Skywalker screams it when the player dies during the Space Battle Over Coruscant level.
Star Wars: Bounty Hunter, when an enemy falls off a ledge and in the second ILM made cutscene.
Star Wars: Dark Forces, when you fall of a cliff.
SWAT 4, sometimes when the VIP is killed in the "VIP Escort" multiplayer mode.
Command and Conquer, used in game, quite often at that.

Other
The Decline of Video Gaming 2 Flash animation features the Wilhelm Scream, along with the caption "pain".
Runaway Brain: At the start of the cartoon after Minnie says, "From now on, you can date your stupid video game," Mickey's video game character lets out a Wilhelm when he dies.
Ratchet and Clank: During all of the Ratchet games apart from Deadlocked, you emit a Wilhelm if you die by falling into oner of the game's many abysses and fissions.

Heheh, kind of interesting. There are a few sound affects i know that i've hear lots and lots of times, got them memorized.

Yeah, there's that sci-fi door opening sound, and this electrical crackle "KssShh...zzzz" that come up ALL the time. And a few explosion sounds. I never noticed the scream before...I bet I will all the time now:p

From the Wiki article:

The Wilhelm's revival came from Star Wars series sound designer Ben Burtt, who tracked down the original recording (which he found as a studio reel labeled "Man being eaten by alligator"). The recording was actually from a film from 1951 titled Distant Drums.

Yes, before i came into this thread, i had every movie the Wilhelm scream had been used in memorized, ingrained in my brain forever :p

I believe it had to do something with an astronaut. Anyway, the person found it very relaxing to be vibrated at that frequency, but ended up having to rush to the bathroom.

This is kinda like that creaking door sound effect, I heard it in Elder Scrolls: Daggerfall of all places, plus about a dozen movies over the years. Thought that was always neat, all of these high end movies using the same sound effect that a little video game did :)
