how come when I turn off waiting for horizontal v-sync on my Prophet 4500 there's no difference in frame rates??
isn't it supposed to make a substantial difference?

Which game?
If its Half-Life or any mod using it, that is because its limited by the software to a set rate.

It doesn't alter the frame rate at all. All it does is put a cap on how high the frame rate is, which is whatever your refresh rate is. If you see no difference, be happy, your frame rate is still high, and the game looks better.
