How does this happen... and keep happening? I've come across several like this, with a telephone pole or a curb in the middle of the driveway. Telephone poles in the middle of the road.

Keep in mind they don't show the steel cable up above for your hands. (you can catch several glimpses when they slip up)
Still scary but orders of magnitude less than without secure hand-grips would be.
