I am guessing the "unrecognizable" part is exactly what he was going for.

His face looks pretty porky, sort of Santa-clause like. But he doesn't seem to have the gut to go with it. So my guess is liposuction. Liposuction on the face is really tricky.

I used to work with this guy who was moderately overweight, what you'd probably a dad-bod today. He apparently had full body lipo but he still had a pudgy face and major league double chin.

Dave is probably working out to avoid going back for another procedure.
