The answer is really that it all depends on your phone and carrier...if you're with verizon, its the toughest.

Verizon use to be a pain in the arse, I have a WM5 phone so Verizon can't do much to try to lock me out. But, I think even the newest models they offer with the most crippled features. You can still create a ringtone, or a wallpaper and email/txt it to yourself. Verizon addy would be yournumber@vzwpix.com make sure the ringer is less than 300kb, send it to yourself and you can save the attatchment and use it as a ringer.

I personally like BITPIM, works with a lot of VZ phones and makes it really painless *shrug*

I have the new chocolate and didn't have a problem with sending myself a 400+kb ringtone that I made. I saw on Howardforums that it needs to be less than 300 also, so I'm not sure what the deal was.

If you're with Verizon (not op, but anyone else), you should check out Howardforums for ways to tinker with your CDMA phone.
