PPO tends to be quite god at covering a wide range of stuff. I used to do work billing insurance for a PT Clinic.

The doctor who performed the proceedure would have a record. No other doctors would have record, unless she asked specifically for it to be sent to them, which is highly unlikely. And no one can access that record without the patients writen consent. It's also highly likely that she would not mention it to anyother doctor so it won't even show up in her history.

I once had a woman tell me she was pregnant, then against my urgings she says that she had an abortion. I guess I will never really know.
