Is my Video card dead?

Here's a pic of my screen.

Well when I reset my PC the problem goes away, but everytime I start the computer this thing happens 50% of the time.

I had this problem even after I formated and installed a new Hard Drive(WD Raptor).

I don't have an extra AGP video card to test it only PCI, Could be the video card cause I changed the Bios once with the enable OC Bios for the Radeon 9500 Pro.

I also did remove the heatsink once to install Arctic Silencer 3 then I began to see this problem I think, now it's back at stock cooling.

When I play CS:Source my monitor turns green sometimes( the map and the charachers look green) then it turns back to normal.

I have the latest 5.7 Catalyst drivers.

Video card is at stock speed.

that dont liik right. turn it on and wait a few minutes then feel the card, be careful, if it is hot to the touch and you pull your finger away after touching it then it is overheating, (vram)
try the pci slot if you can find a card to fit check for artifacts

open 'er back up and see if you f'ed anything up when you originally took it apart
 
Could be several problems:
Drivers are corrupt
Heat issues
Did you use arctic silver 3 with your silencer? If so, it conducts electricity, and may be shorting out some things.
 
Funny, I have that same problem with my 9700 pro. It worked fine for about two years and then suddenly the screen started messing up just the same way yours is. I think it might have to do with heat (at least in my case), cuz when I took off the left side of the case and pointed a 12" oscilating fan into the case, there were no problems with the graphics card. Then I turned off the fan, and eventually the display would become corrupt. Turn it back on, change resolutions or some desktop settings, and the card would function just fine.

I don't know why after two years the stock cooling wouldn't be enough (yes, the fan is spinning), but if anyone has any other ideas as to why the card is malfunctioning, or can suggest, a small, and preferably cheap method of cooling the card, it would be greatly appreciated.

I fixed it, it was heat issue I reinstalled the Arctic Silencer 3 with AS3 on the core and everyting is stable now. 😎
