Is my PSU dying?

If you are getting that reading by useing a Voltage meter on a spare connector then yes. If your getting that reading from software then who knows. Software voltage readings can not be trusted to be accurate. If you want to check the voltage then use a DVOM on the yellow (+0 and black (-) wire of a spare molex connector. If the reading is outside the tolerance of +/-5% then the PSU needs to be replaced.

If you're reading that voltage in the BIOS, I think I'd be concerned. The BIOS should be reasonably accurate and that 11.18 is more than 5% low, which is low for the main voltage rails. Buy a PSU tester at Radio Shack, costs about $10 or so, test you PSU. Or buy on-line.
