Is an add-on soundcard worthwhile for games?

Not sure this is the right forum, but no other choice...

I have been using the onboard realtek audio for years now, but I have been pretty casual. I am getting back into gaming and have noticed some deficiencies of the onboard audio, like buzzing/crackle sounds when there is not audio playing or hitching in some games (not sure it is audio related).

Is getting a soundcard worthwhile? What is a good one to get? (PCI-e, I generally use headphones)

For most people, no. On board audio is good enough now that unless you're really into it there is no reason to do it. The issues you describe are more about your particular setup than onboard audio in general.
