I'm in safe mode. What now?

Windows XP home is booting to the log in screen and then going to the desk top, but no icons. It just sits there. I have this in safe mode, safe mode is in all 4 corners of the screen, and hit yes, enter, then what? I guess I'm suppose to fix the problem, then restart. But what's the problem and how do I get there?
 
Sometimes entering SAFE MODE takes a while (5-10 minutes is not unusual).
