Im at 30hz instead of 60hz... help

Im at 30hz.. I installed and uninstalled nvidia driver as well. The native resolution is 4k I will play games @ 1080p.

but first we need to fix this 30hz,

FYI, I have display port to display port and hdmi to hdmi ,,,
