Yeah, I was just watching the end of Part II on AMC... where Connie goes to Michael and pleads for Fredo's life. Then Michael hugs Fredo and gives the "signal" to off Fredo.

Fredo was just incompetent and also unwilling to accept his incompetence. Had he been willing to go take a small or no role in the family business, he likely wouldn't have fouled up. But he wanted some side action himself and he resented being upstaged by his younger brother. Simply put, his abilities didn't match his ambitions. He was never disloyal to the family, merely incompetent.
