I have a boss who likes to say, "Why didn't you handle that?" to any issue that comes up with clients regardless of cause. If it's an issue, then damnit I should've prevented it somehow. I'm supposed to recall every bit of information of every project I've ever worked on, on demand.

What a great way to destroy employee morale. No matter how hard you try, you're going to be in the wrong.
