Can't find the right words to explain this:
problem

Everything is clean, virus scans, spyware scans, hijackthis, combofix, etc. etc. etc.

I can use tweakui to "fix" the icons, but after a reboot or three they're back the way they are.

Just on cds & dvds, *all* cds and dvds, but not on the hard drive, floppy, network shares, just the cdrom.

Windows XP Pro all updates, IE7 all updates. Only recent driver change was going from an nvidia graphics card to an ati graphics card...

looks like the icon library is corrupted. I'd have housecall run a full and complete scan of your system

That tells me that the file association in your system for CDs is IE7. IOW, when you put a CD in the drive, the system has been told to open the files in IE7.

Open My Computer, and right click on your optical drive, select Properties, and for each actegory (Music CD, DVD Movie, etc.) select TAKE NO ACTION. Saved and exit and see if that changes anything.
