Identify this Motherboard

I'm currently working on a machine that was built by a local shop and I cannot for the life of me figure out who makes the motherboard. I'm leaning towards biostar but I can't find a picture on their site. And before you ask, yes, I'm sure there is no model number on the board. Here's the best I have so far:

CPU: Athlon 2800+
Chipset: Nforce 3
PhenoixBios
1x AGP(x4)?
5x PCI
And one odd sound card thing that plugs into a proprietary slot on the Mobo

If this boots, you can extract the manufacturer from the BIOS ID string (see bottom of POST screen).
 
It boots and runs fine, I'm just missing some drivers.

Anyway, I tried looking up the Bios ID but either I couldn't find an accurate list or the Phoenix AwardBIOS ID doesn't list the mobo manufacturer.
