Identify mystery dialog - cannot tell which program it's from

For a week or so, I've started seeing this weird blue dialog. It appears periodically, has no title bar or any identifying characteristics, and the error message is useless. I can't figure out which program it's from — the one app I updated last week has been confirmed not to be the source.

Can you spot which program this is from? Or is there a utility that monitors dialog open requests and would pinpoint the source of a dialog?

Open Task Manager and force quit things until it goes away. Whatever the last thing you "end task" for, that was the culprit.

Then boot from an AVAST recovery CD or something and run a virus scan, 'cuz that one's weird lookin'.

Install Sysinternals freeware Autoruns for Windows v11.42 which among it's many functions lets you go straight to the corresponding registry key, lists the publisher, and full image path for any entry. For quickly and positively identifying and disabling problematic, unknown or malicious processes it's one of the most valuable diagnostic utilities I use.

From Sysinternals:
This utility, which has the most comprehensive knowledge of auto-starting locations of any startup monitor, shows you what programs are configured to run during system bootup or login, and shows you the entries in the order Windows processes them.
