I want to add a file to a .MDR file, what program should i use?

I have a large file, ending wit .MDR.

I can open the file with winrar, and see all its content, but i wanna add another file into that .MDR file and resave that .MDR

Winrar doesnt allow to add/save if the file doesnt have a .zip or .rar extension.
