I need a rebate form. Please help!

This goes out to all SoCal residents who have recently shopped at Fry's. I don't know if anyone can help but here goes. I just recently bought a 17" monitor from Fry's (it's in the ad as the $127 - $30 rebate on the front (top right) of the ad for this weekend (March 16-18). However, the sales clerk did not give me the rebate. I'm hoping that one of the members of the forum (who bought that same monitor and got the rebate form) would be so kind as to please send me a copy of it. Thanks.

It might help if you at least tell us what kind of monitor it was. Another thing you can try is going to the manufacturers website to see if it's there.
