I have an ATi graphics card from 1991 (ISA). Can someone help me identify what it is?

Has a sticker on the bios chip that says "GRAPHICS ULTRA/A"

Chip says

"ATi 38800-1
1568F0002
JAPAN 9206AAA"

Has 2 small chips that say

"45.0000 MHz
V92 A"

and

"33.0000 MHz
V92 B"


Anyone have a clue? When did ATI start making cards?
