It's from stuffing all the food into its pouch inside its mouth. I used to own several hamsters growing up and they're fun to play with.

doesn't that predispose it to getting an infection with all that food in there?

This is a creature who has evolved over millions of years to hold a ton of food in its cheeks... Mother Nature has already solved that one.
