Yup.
Did you ever move the library to another computer over a network? iTunes annoyingly keeps track of those movements and updates it's self. Possibly what happened there.

The only way that I know of to fix it is by reimporting the files :(

I did not move it. I cloned it to a new drive, then replaced the old drive with the new one.

I hate it too on some songs:

but in itunes it says:
/Volumes/Machintosh HDD 2/Music/Music/Apocalyptica/7th Symphony/01 At The Gates Of Manala.mp3

You could do a find in replace on the
iTunes Music Library.xml file

I'm stumped :\

Ok, so I fixed it. I moved the iTunes folder from the internal HD to the external (which I was not required to do when this was first set up). After a little cleaning up, everything seems fine.

Well, except for one minor thing: Many of my apps are missing their icons in grid view (which is what I use). They are showing up as blank apps. However, for many, if I do a 'get info', the icon reappears (many, but not all). Not sure how much time I am going to invest in trying to fix that issue unless I get bored. ;)
