How can I create this effect? So awesome!

Is it a specific lens? Is it a photoshop thing?
Its blowing my mind and I don't know what to search to do it.

Via lens, tilt-shift or perspective control. Yes you can do it in post as well.

That's tilt shift. You have a few options, so I can list them in cost from least to greatest.

• Buy an actual TS lens - EXPENSIVE!

You can easily do this in photoshop too. Basically you blur the top and bottom, feather it into the middle where its in focus. Plenty of tutorials out there that go into more detail.

Yah, this is something to remember - but the post processing methods still get pretty close.

lol....long ago, before digital, if you wanted this type of effect you would use a light coating of vaseline on a filter, to blur out the areas, leaving a "target" area sharp. still fun to experiment with. http://www.photographyblog.com/news/stealth_gear_square_filter_system/
