How to get Power of Attorney document?

My older brother is physically handicapped and he has collection agencies calling him asking for some kind of payment. I just talked to a law office and I told them I have power of attorney over him.

She asked if I can fax her the document stating this. Does anyone know where I can get a free legal PofA document so I can talk to them about this account.

Seems like spending $13 bucks is not such a big deal if it'll help your brother get away from financial trouble with collection agencies.

My thought was that you simply type the info and submit that.
