Mrs. will assume she's married and offend her.
Miss will make her seem like a kid and offend her.
Hey Sexy will assume she's sexy and offend her. ;)

Madam if you know she's older than you, Ms if she's around your age, and Miss if she's a lot younger than you. Of course you can't go wrong with "Dear tits" ;)

edit: when I say old, I mean grandma old, don't go calling woman in their 40s madam unless they run a brothel...
