I never do "regular" maintenance per se: I don't ever run antivirus or anti-spyware, I hardly uninstall software and I never defrag.
Why?
I never run executables or stuff from sites that I do not trust.
I have latest windows patches.
I keep browser updates.
I don't go to sites that I don't trust in general--stick to mainstream stuff and research-oriented.
I don't install software that I won't need.
I don't put lots of large files on my drive to fill it up and then later remove them.
I have few tools, but they are ones that I know I will use again and again. I very very rarely go download stuff to try it out then uninstall it. Most of my computer use is: Email, IM, web, lots of programming, typing documents and plenty of research.
I typically rarely have more than say 500 MB of user content on my drives (beyond the OS and tools).
I DO regularly perform "maintenace" on my inbox, desktop (i tend to leave lots of post-it style .txt files on there), my bookmarks and my documents folders. I find that things accumulate often in those 😉
While I could perhaps stand to defrag once in awhile, the performance increase has always been negligible in my experience.
