How loud is a Dremel when using it to cut into a case?

Mine has variable rpm although I have no clue why noise is an issue.. the higher the rpm the more accurate the cut will be, or at least faster and less dangerous if you are using a cutting wheel because otherwise it'll melt or break off if used at too low an RPM..

Those are a must. So noise doesn't matter then with earplugs in.

The noise level depends a lot on what you are cutting and the surface. For example, if you stand a case panel up, you will hear a ton of reverberation.

The noise is annoying at the very least, but bearable without earplugs. But there's no reason not to pick up some ear plugs with your goggles and mask (you DO NOT want that red dust in your sinuses or lungs, trust me!).

Lowering the RPM won't do much for making it quiet, and if you try to go too slow while cutting the metal you could harm the dremel's motor or at least have a bad time trying to cut the case.

The real answer though is yes, cutting a case with a dremel at 3:00 a.m. will always piss off the neighbors.
