How long do you have till your RMA no. can't be used?

Okay here it is.

I got a reply back from "ATI" telling my where to ship my video card to for the RMA, now that was back in ..." 4 Apr 2006 ".

I know i should have sent it then but i got tied up with my studies and stuff, now what i want to know is, should i contact ATI and get another RMA form filled out or can i just use the old one(the RMA number i already got) from " 4 Apr 2006 ".

What should i do?

EDIT: Also what amount should i insure the video card for?

Probably best to contact ATI again before you ship anything out.
