How does one obtain one's "medical history?"

So I'm looking to get my teeth cleaned at a local JuCo dental hygiene program clinic (I have no dental insurance, and I like to live on the edge). They require my "medical history" for the initial consult. However, I have no idea how to obtain this.

My family's not big on doctors. In fact, I haven't been to a physician in probably 9 years, for my pre-college checkup. Before that, I only ever saw a doc for the yearly athletic physical. And even then, it was completely random where I went. The last time I had a regular doctor was probably before my teens.

Anyway. So my question is, wtf do I get any kind of records of my medical history if I've never had a regular GP, and the times I did see a doctor, I don't have any idea who the doctor was or how I'd find out and get in touch. Is there some kind of central repository for medical information? Like is it all kept in a County record-house or something? Or do I have to go back and do it all over?:laugh:

Your "medical history", is comprised of all the 'attending physician statements' (charts) from all of the doctor/hospital/clinic visits you have had in your life. You can order your charts from those providers. It generally takes some time, especially if they have a copy service. You may have to pay for them, and fill out forms.

There is no central repository for medical records, thank goodness.

Are you sure you didn't misunderstand? Usually they just have you fill out a form asking you questions about your medical history. I don't think it means you have to get the exact records from previous doctors.

See but this my problem. I don't know who the providers are. It would be roughly akin to me trying to get my automotive service records by contacting all the places I've had my oil changed.

Google Health They can automatically import your medical information from certain health providers. There are few providers now, but I bet it will grow.

Google Health They can automatically import your medical information from certain health providers. There are few providers now, but I bet it will grow.

Wow. I will NOT be importing my medical records.

I doubt you will need them unless you have something the dentist needs to consult your chart over.

If that turns out to be the case, you will just have to do as good as you can, or go see a non-NAZI dentist.:laugh:

Yeah, I'm guessing you guys are right. Officially, it says "we'll review your medical history" so I assumed they'd want papers or something. But yeah, maybe I'll start keeping copies from now on.

Oh and that Google Health makes me happy I don't have any records to import. :laugh:
