How does one get a CableCard from Comcast?

1. Does one need to switch to the service first (Cable Package X) and then request one or can one request one and then switch to the service?

2. Can you force Comcast to give one to for self-install or do they require sending a technician to install one for you?

3. Do they charge for one?

Based on a phone conversation with them they don't seem to make it so easy.

I went to the local Comcast office. Told them I wanted a cablecard. They gave me one. Once I installed it, I called their special cablecard number to get it functioning. Took three calls but it was less aggravating than I expected.

Edit because I totally didn't answer your questions:
I don't know but don't think you need any special package.
Dunno about forcing a self install but I had zero issues just getting the card.
I pay something like a dollar or two a month. Much cheaper than additional HD cable boxes.
