How Does Dual Slot PCI GPU's Venting Work?

Does the fan inside the case suck air from the case and blow it out the back? Or is it the other way around?

I'm planning on installing a 120mm fan at the back of my case below the card, so will it benefit me to blow air in or outside my case with a dual slot card?

Why are you wanting to put a 120mm fan below your card at the back?

I only have 1 120mm fan that blows air out my case near the CPU and that is at the top back portion of my case which doesn't reach my GPU.

I'm thinking of upgrading my card to something like the GTX 280 or 295 and I'm worried that I'll need extra cooling. My case doesn't have any front or side fans.
