How does a company sell a 'refurb' OS?

I was just browsing the refurb section of several websites. they have refurbed operating systems. How can an OS be refurbed? I'm kind of confused here...

The software (in a box) was probably returned by whoever originally bought it. So they shrink-wrapped it up and are selling it again as 'refurbished'. Sounds ok, except I guess the CDs could be scratched...

they just add a few more bugs and then rewrap it in plastic.
