how do you grease a bike?

I've really haven't taken care of the grease on my bike chain. it probably barely has any left on it now because my pants cuffs always brush against the gear and pick up grease.

1. how do I grease a bike chain?
2. is there anything special I should do since I've been riding on it for a while with very little grease?
3. what kind of grease should I use? (is there like a generic type that I might have laying around the house, or should I get a certain brand at the hardware store?)

You do NOT use grease on a chain!
