my 2 week notice for tim horton's consisted of a post-it note stuck on the manager's door, along with the 50 million other ones explaining absences and asking for time off.

I write mine two weeks after I quit. Never had to write one though.

You don't HAVE to give notice, but you really should. You don't wanna burn bridges.

Dear Boss,

Due to an improved job oportunity I must tender my resignation with this company. I have enjoyed working here and with the people I have worked with. It would be possible for me to stay for up to three weeks to train my replacement but if not my last day will be on --/--/---.

Sincerely

I like yllus's better but then I've worked for the same company over twenty years and hadn't written one before.

Pretty sorry excuse ehh.

To: Whom it may concern

I'm resigning, this is my two week notice, but I am happy to say that I can get out of this company much faster than that. If you find any suitable replacement before my two weeks are up, please let me go ASAP.

Sincerely,

Employee of the aforementioned statement.

Never written one, always said it in person.

Some employers prefer / require that you give a written notice. Especially corporations who need it for auditing and small business who don't want to contend with employees that claim they were fired.
