If the name is Casey Jones, in a formal cover letter, do I write Dear Mr/Ms. Jones or do I just write Dear Casey Jones?
 
I'm unsure of your situation as you didn't mention it, but I'll assume you're writing to someone in a more superior position. Call his or her office and ask the office assistant what the person's gender is.
