How do i write a 30 day notice?

I need to give a 30 day notice for my roommate and I. Is there a standard template for writing one? Don't have much experience with this. Any suggestions would be great since i know many of you have dealt with this before.

To Whom It May Concern.

This is to inform you in writing per the contract we signed on ++++++ that we are moving on ++++++++ which is the end of our contract agreement. If you have any questions about this move please do not hesitate to ask.

Make a copy and get it notorized when you sign them so there wont be any confusion as to you not handing one in. This way if they decide to play around to hold your security deposit and say you never gave notice you have the second copy.
