How do I send in an ammended return for 2002?

I posted here (I'm at top, followed by a response, followed by me, followed by a response) about an RRSP I've had in Canada. Apparently since I'm now a US tax resident I need to report the existence of this RRSP (like a 401k for those following along).

In short I have to file a 1040x which from irs.gov looks to be an ammended tax return. Now since all my numbers will stay the same (income, overpayment, medicare, etc. etc.) I am not asking for more money, or trying to pay IRS anymore. I'm merely reporting that I have this RRSP and the amount of it, and I'll send in a photocopy of my statement. Where on the 1040x would I put that specific info and can I leave the rest of the form blank? Do I simply drop in those two pages and my photocopies in the mail and be done with it since they still have my original 2002 return...?

Should I just fill in name, SSN, etc. and then A,B,C, sign it, then go to part I? And would the "number of exemptions" be the dollar amount I'm exempting? Plus it's in freaking canadian currency wth! Fvcking RRSP. What a hassel!
