There should have been a link to open the mail-in rebate form on the product's page of the store it was purchased from, so you could print it out. The instructions for submitting the rebate are on that form(including the address to mail it to). Most rebates are similar (proof of purchase like UPC code, copy of receipt showing the product along with the place & date of purchase, and a filled out rebate form). It is also recommended to read the fine print as some rebates require special instructions like circling the product purchased on the receipt, etc.

<---Just filled out and mailed in a rebate for some Corsair ram this morning. :)

I think you mail the whole case back to Coolermaster, they put a $20 bill inside and mail it back to you. Its really the only way to verify that you actually own it.

Not necessarily, they may have extended the offer and changed the dates on the rebate form. If a rebate was showing on the product page at the time the item was purchased, you could try calling/contacting newegg on monday to see if they will email you the previous version of the rebate form.

Like Directron did with the rebates below:

Old Form ended 10/22

New Form started 10/23
