i have from what i see, a paper entitled
Identity Theft Survey Report

for the United States, prepared by Synovate, for the FTC. seems like it should be like:
United States. Federal Trade Commission. Identity Theft Survey Report. Synovate (date) or pretty similar to that, am i correct?
