How do I capture what I see on my screen?

Hello I dont know where to look for a spacific capture card that I need I want to be able to record what I am seeing on my/someone elses computer screen so that I can use it in adobe premier, did that make sence?
Where can I get a video capture card that will allow me to do this? I see plenty that will allow me to capture cable or rca but none that have monitor/vga or even digital inputs.......

PS I want to capture action, not just a single frame

Find one with s-video in. Use a video card that has s-video out. Then record the s-video.
