how can you tell if someone's behind a NAT?

I read that Comcast is cracking down on people who've implemented a NAT in their home network.

My question is... how can they tell if you are using NAT?
 
My ISP (hickory tech in MN) accually set up my ROuter for me!

The only way I could think of that they could is if they trace the MAC address. But you could spoof it! I would think If your router has a firewall they would be unable to track the number of comps on the Connection.
