How bad is installing a PCI card in a running motherboard?

Having never done this in all my years I have no idea what occurs. A co-worker of mine just mentioned that she did while installing a PCI wifi card, caused the PC to lock up and now the card is recognized in Windows but is signalled as not working properly in Device Manager.

Is there any chance it survived?

Some people should stick to calculators 😉

doubt it. u should usually not open the case with the computer on, but upgrading? only when it's off. atleast she learned a lesson.
