How are TV ratings determined?

Obviously it's by the number of people who tune in to watch it, right? But is there a minimum time you have to stay on the network for it to be counted towards the ratings? I mean, when I'm channel surfing, staying on a network/show for 10 secs, do I count towards the ratings of that show?
 
No, only families in the Nielsen program count. AFAIK it's around 2000 families with a little transmitter box that monitors what they watch. They are given very specific instructions for their TV watching habits.

There's no way anyone can tell what YOU are watching. They just take a sample of the ones who participate.

During "sweeps week" polls are conducted to random TV viewers about their TV watching habits. That provides a bigger picture of who is watching what.
 
Oh ok, but I still hear things like "750,000 viewers tuned in last night to see blah blah". I was just wondering how it worked.
