I know that Cricket's $45/month plan is unlimited everything. Their "android" plan is $55/month though. They're not prepaid in the sense that there aren't buckets of minutes that you fill with cards, but there's no contract. I'd also call them "super-regional" where roaming coverage does require pre-paid minutes, but covered cities are all included.
