hospital accidentally injects woman with cleaning fluid

Toxic anti-septic cleaner? Well, all I can say is - at least she died with clean insides.

Now, on a more serious note, this is a very unfortunate event.

 
I'd love to know what the bottle/container that cleaning solution was in looked like.

There are numerous errors because of look alike bottles & drugs, but this one looks pretty bad:Q
 
Ok...how the phuck do you confuse the medication with cleaning fluid. If you think about it, somone had to put the cleaning fluid in the syringe...

omg. this is horrible.

when i was in india they had incompetent nurses who were about to inject me with a needle that had not had air bubbles removed.

I didn't take it as mocking...i took it as him just saying that one is one the way because this should never happen.

well i think they had the confusion before they put it in the syringe... so they thought they were putting something legit into the syringe.

but yea... what a horrible way to go 🙁

Why didn't they do this before? Did it cost more? Did they have to wait till someone got killed before they decided to pay a bit more for safer tools?

And what kinda protocol did the hospital have so that a toxic fluid like this could end up injected into a patient by accident? It might be just one person's fault, but perhaps the quality/safety department would have thought of this possibility and put protocols in place to minimalize accidents like this.

Your body can tolerate a whole lot more air than that... I'e seen ~20 ml injected into a vein without a problem...

That stuff in the movies is a myth...
 
I'm shocked that they openly admitted the mistake and are taking full responsibility for it.
Shame on them for the screw up, but you have to give them points for being decent enough to do the right thing and fess up.
You don't see honesty following liability very often.
Classy move on the hospital's part.
 
I can see injecting the wrong medication, but toxic cleaning fluid?

Somebody worked his last day in medicine...
 
How in the heck did somebody confuse the radiation dye stuff with what is basically bleach?? Somebody needs their licsense revoked.
